USDA Swine Tag Request Form

Please complete the following form to submit a request for Allflex RFID Swine Tags fulfilled by Merck Animal Health. This initiative to provide no-cost RFID tags for swine was awarded by the U.S. Department of Agriculture’s (USDA) Animal and Plant Health Inspection Service (APHIS). The goal of the initiative is to boost national swine disease, traceability, which is vital to supporting the safety and marketability of the U.S. swine herd.

What is unique about this program is a direct, manufacture-to-swine premises, distribution model. Merck Animal Health will receive, process, and ship orders directly to producers, State Animal Health Organizations (SAHO), and Exhibitions, removing APHIS as intermediaries and expediting putting tags directly into producers’ hands and pigs’ ears.

To order these tags you must have a valid premises identification number (PIN) and provide contact and shipping information. Commercial Sow Farms need to provide the number of sows onsite for that premises ID location, SAHO’s will need to provide the number of sows (female breeding age pigs) in their state, and Exhibitions, the number of pigs registered for the show.

Allflex RFID tags will need to be applied with the appropriate Allflex RFID tag applicator, which is not supplied through this program. Please contact your preferred animal health distributor.
